About the taskforce

The Bone and Joint Decade 2000-2010 Task Force on Neck Pain and Its Associated Disorders is comprised of a group of international clinician-scientists and methodologists to undertake a best-evidence synthesis on neck pain and its associated disorders. The mandate of the Task Force is to make recommendations of clinical practice guidelines for the management of neck pain and its associated disorders. The Task Force will develop the guidelines based on:

A systematic and exhaustive review of the literature on neck pain and its associated disorders.
The original research to be conducted by the Task Force on risks associated with the treatment of neck pain and on formal decision analyses of treatment.
The consensus of the top experts in the world in the absence of scientific evidence.

The findings will be collated using best-evidence synthesis, which addresses risk and prevention, diagnosis, prognosis and treatment risks and benefits. The final written report of the Task Force will be published in a major health-care journal.

Workshop

Governance

The structure of The Bone and Joint Decade 2000-2010 Task Force on Neck Pain and Its Associated Disorders is designed as follows:

The mandate of this committee includes making major decision regarding the conduct and work of the Task Force and tracking expenditures. The Administrative Committee is responsible for recruiting Scientific Secretariat and Advisory Board members, providing financial and progress reports to the funding agencies, and ensuring timelines and budget guidelines are followed.

Scott Haldeman, D.C., M.D., Ph.D., F.R.C.P(C)
President and Member of the Scientific Secretariat

            Scott Haldeman is an adjunct professor in the Department of Epidemiology at the University of California, Los Angeles, a clinical professor in the Department of Neurology at the University of California, Irvine and an adjunct professor in the Research Department at the Southern California University of Health Sciences. He is a graduate of the Palmer College of Chiropractic and the University of Pretoria. He holds a Ph.D. degree in neurophysiology and a M.D. degree from the University of British Columbia, Canada and completed his residency in neurology at the University of California at Irvine.

            He is the past President of the North American Spine Society, the North American Academy of Manipulative Medicine and the Orange County Neurological Society. He is Chairman of the Research Council of the World Federation of Chiropractic and has served on the Executive Council of the International Society for the Study of the Lumbar Spine. Dr. Haldeman is the Section Editor for Non-Surgical Care for The Spine Journal and sits on the editorial boards of Spine, Journal of Manipulative and Physiological Therapeutics, Journal of the Canadian Chiropractic Association, Journal of the Australian Chiropractic Association and Alternative Therapies in Health and Medicine.

            He was a panelist for the Clinical Practice Guideline on "Acute Low Back Problems in Adults" established by the Agency for Health Care Policy and Research, and was Chairman of the "Guidelines for Chiropractic Quality Assurance and Practice Parameters." He has been in private practice as a neurologist since 1980, with an emphasis on spinal disorders has managed a multidisciplinary group of physicians that included neurologists, psychiatrists, psychologist, occupational medical specialists, physiotherapists and chiropractors.

            Dr. Haldeman is well respected in the field of spine care. He has the management experience necessary for the role of President of the Task Force on Neck Pain, and has the medical and research expertise to ensure his value on the Scientific Secretariat.

J. David Cassidy, D.C., Ph.D
Scientific Secretary and Member of the Scientific Secretariat

Dr. Cassidy is a senior scientist in the Division of Outcomes and Population Health at the Toronto Western Hospital Research Institute and holds the Research Chair in Artists' Health within the University Health Network. He is currently a Professor of Epidemiology with the Department of Public Health Sciences in the Faculty of Medicine at the University of Toronto. Dr. Cassidy was formerly an Associate Professor in Epidemiology at the University of Alberta and is currently an adjunct professor in the Department of Public Health Sciences at the University of Alberta. While in Alberta, Dr. Cassidy was a Health Scholar with the Alberta Heritage Foundation for Medical Research and an Associated Scientist with the Alberta Centre for Injury Control and Research. He currently is a Guest Research Professor with the Section for Personal Injury Prevention at Karolinska Institute in Stockholm, Sweden. Dr. Cassidy has also held the positions of Research Director for the Institute for Health & Outcomes in the Department of Physical Medicine and Rehabilitation and Research Director for the Division of Orthopedic Surgery in the Department of Surgery at the University of Saskatchewan, he held professorships in the departments of Surgery and Rehabilitation Medicine.

Originally trained as a chiropractor, Dr. Cassidy practiced in both Ontario and Saskatchewan, where he was a member of the Medical-Dental Staff at the Royal University Hospital and a consultant chiropractor to the University of Saskatchewan's Student Health Centre. He also holds a Bachelor's degree in Anatomy, a Master of Science in Surgery and a Doctorate in Pathology from the College of Medicine at the University of Saskatchewan. He has graduate training in Epidemiology and Biostatistics from Tuft's University in Boston, the John Hopkins School of Public Health in Baltimore and from Erasmus University in Rotterdam. Dr. Cassidy has authored over 120 scientific papers in books and journals, including publications in the New England Journal of Medicine, Spine, Medical Care, Clinical Epidemiology, Accident Analysis and Prevention, the Journal of Rehabilitation Medicine, the Canadian Journal of Public Health and the Scandinavian Journal of Public Health. his research interests include musculoskeletal and injury epidemiology. He is scientific secretary for the World Health Organization's Collaborating Centre Task Force on Mild Traumatic Brain Injury and the Decade of the Bone and Joint 2000 - 2010 Task Force on Neck Pain. He is also a member of the expert working group on mild traumatic brain injury at the Centers for Disease Control and Prevention in the United States.

Åke Nygren, D.D.S., M.D., Ph.D.
Vice President

Dr. Nygren received his Doctor of Dental Surgery from Karolinska Institute in 1963, his medical degree from Karolinska Institute in 1974, his licentiate of Medicine and Surgery from Barcelona in 1978 and his Doctorate Degree (Dr. Med. Sc.) from Karolinska Institute in 1984. He is a professor at Karolinska Institute, and has been the Head of the Section for Personal injury Prevention in the Department of Clinical Neurosciences at the Karolinska Institute since 1994. Dr. Nygren has been a part of several government research projects in Sweden investigating issues such as hospital planning for disasters, traffic medicine, and the outcome of trauma treatment. He was a member of the Scientific Secretariat for the Quebec Task Force on Whiplash Associated Disorders and a member of the Task Force on the Effects of Rehabilitation. He is currently the Vice President of the World Health Organization Collaborating Centre Task Force on Mild Brain Injury and is an executive board member of the World Health Organization Collaborating Centre for Neurotrauma at Karolinska Institute.

Dr. Nygren is a well-respected researcher in Sweden. His close ties with the Swedish academic community, medical community and Swedish insurance system will be of value in communicating the Task Force findings to these stakeholders.

Jon Schubert, C.M.A.
Vice President

Mr. Schubert provides private consultative services to the insurance, health and rehabilitation industries, with particular expertise in the areas of strategic thinking and implementation of disability management, outcomes research, rehabilitation programs and underwriting. He worked at the Saskatchewan Government Insurance agency (a Crown Corporation which provides province-wide automobile and motor vehicle injury insurance) for over 20 years, and was the Assistant Vice President for Claims from 1991 until 1999. During this tenure, he introduced a new traffic injury insurance plan in Saskatchewan and oversaw the implementation of rehabilitation programs for brain injury, whiplash and other traffic injuries. He also implemented evidence-based outcomes research to evaluate the effectiveness of these rehabilitation programs. He is currently the Vice President of the World Health Organization Collaborating Centre Task Force on Mild Traumatic Brain Injury, and was the Insurance Consultant to the Quebec Task Force on Whiplash-Associated Disorders. Mr. Schubert has provided expert testimony for a number of commissions, served as a consultant to insurance companies in Scandinavia and presented papers at international meetings on rehabilitation and insurance.

Linda Carroll, Ph.D.
Member of Scientific Secretariat

Dr. Carroll is an associate professor of epidemiology in the Department of Public Health Sciences and the Alberta Centre for Injury Control and Research at the University of Alberta. She is an adjunct professor in the Department of Psychology and the University of Saskatchewan and is a research consultant in the Section for Personal Injury Prevention at the Karolinska Institute in Sweden. Her background as a health service provider in clinical psychology includes chronic pain management, and psychological services to individuals with medical illnesses and mental health disorders. Her primary research focus is on psychological factors in pain and health, recovery after injury, patient-centred health outcomes and systematic reviews. In addition to being a member of the Scientific Secretariat in this task force, she is also a member of the Scientific Secretariat on the World Health Organization Collaborating Centre Task Force on Mild Traumatic Brain Injury.

Pierre Côté, D.C., Ph.D.
Member of Scientific Secretariat

Dr. Côté received his Doctorate in epidemiology from the Department of Public Health Sciences at the University of Toronto. He is an assistant professor in the Department of Public Health Sciences at the University of Toronto, a scientist at the Institute for Work and Health and an adjunct professor in the Graduate Studies and Research Department at the Canadian Memorial Chiropractic College in Toronto. He is a member of the Canadian Society for Epidemiology and Biostatistics and the Society for Epidemiologic Research. His research interests include the epidemiology of disability related to musculoskeletal pain and depression, including the impact of health care delivery on the recovery of soft-tissue injuries. Dr. Côté is the recipient of a New Investigator Award from the Canadian Institute of Health Research.

Lena Holm, B.Sc.
Member of the Scientific Secretariat

Ms. Holm is a sociologist and a Ph.D. candidate in epidemiology at the Institute of Environmental Medicine, Division of Epidemiology at the Karolinska Institute in Stockholm, Sweden. She was also a member of the Scientific Secretariat on the World Health Organization Collaborating Centre Task Force on Mild Traumatic Brain Injury which utilizes similar methods as the Neck Pain Task Force on preparing patient care guidelines based on systematic reviews of the world literature.

Sheilah Hogg-Johnson, Ph.D.
Member of the Scientific Secretariat

Dr. Hogg-Johnson is a senior biostatistician at the Institute for Work and Health in Toronto, and is an assistant professor in the Department of Public Health Sciences at the University of Toronto. She is a specialist in statistical methodology and prognostic modeling and has extensive experience in musculoskeletal research design and analysis. Dr. Hogg-Johnson is the co-recipient of a grant from the National Institute for Occupational Safety & Health (NIOSH) for studying work-related musculoskeletal disorders.

Eric Hurwitz, D.C., Ph.D.
Member of Scientific Secretariat

Dr. Hurwitz is currently an assistant professor in the Department of Epidemiology, School of Public Health at the University of California, Los Angeles (UCLA) and at the Southern California University of Health Sciences. He has a wealth of epidemiologic and health-services research experience with UCLA and RAND, and serves as a peer reviewer for the National Institutes of Health and the RAND-based Southern California Evidence-Based Practice Center. He is the project director of the UCLA Low-Back Pain and Neck Pain Studies, two federally-funded randomized clinical trials addressing the effectiveness of chiropractic and medical care for low-back and neck pain. Dr. Hurwitz is an author or co-author of scientific articles in several biomedical journals, including Spine, Annals of Internal Medicine, Arthritis and Rheumatism, and the Journal of Clinical Epidemiology, among others.

Margareta Nordin, P.T., Ph.D.
Member of the Scientific Secretariat

Dr. Nordin is the Director of the Occupational & Industrial Orthopaedic Centre (OIOC) of the Hospital for Joint Diseases and the Program Director of the Program of Ergonomics and Biomechanics at New York University. She is also a professor at the Department of Orthopaedics and Environmental Medicine at New York University. She has been a panel member of the Agency for Health Care Policy and Research for (AHCPR) Clinical Practice Guidelines for Acute Low Back Problems in Adults (1990-1994), the Quebec Task Force on Whiplash Associated Disorders (WAD) (1992-1995) and the Paris Task Force on Exercises and Low Back Pain (1996-1998). Dr. Nordin was the President for the International Society for the Study of the Lumbar Spine (ISSLS) in 1999-2000. Dr. Nordin has won numerous awards including an award from the Arthritis Foundation (1997 & 2000) and the Grammer European Spine Journal Award for best basic science paper (2000/2001).

Eugene J. Carragee, M.D.

Dr. Carragee is Professor of Orthopaedic Surgery at the Stanford University School of Medicine and the Director of the Orthopaedic Spine Center at the Stanford Hospital & Clinics. In addition, Dr. Carragee is an active US Army Reserve Officer and Command Surgeon for the U.S. Army's 351st Civil Affairs Command. The areas of his research interests include diagnosis, treatment and outcomes for cervical and lumbar spine disorders. Dr. Carragee has won numerous civilian and military awards including the Volvo Clinical Research Award (2000), five Outstanding Research Paper Awards from the North American Spine Society (1997, 1998, 1999, 2000, 2001), an ISSLS award for research (2003), as well as UNTAC/UNESCO Award, UN Services Medal Kampuchea Assignment (1992-1993), and the Medical Service Award (Viet Nam, 1994).

Paul Peloso, M.D., M.Sc., F.R.C.P(C)
Member of the Scientific Secretariat

Dr. Peloso is an associate professor of internal medicine at the University of Iowa and a rheumatologist at the Department of Internal Medicine at the University of Iowa Health Care. Dr. Peloso has a B.Sc. in biochemistry and a B.A. in sociology from McMaster University and a M.Sc. in clinical epidemiology from the University of Toronto. He received his M.D. from the University of Calgary and completed his residency at the University of Calgary and his fellowship at the University of Toronto. He holds Canadian and American Specialist Certificates in Internal Medicine and Rheumatology. His research activities include randomized controlled trials in the area of arthritis and chronic pain. Dr. Peloso is also a member of the Scientific Secretariat of the World Health Organization Collaborating Centre Task Force on Mild Traumatic Brain Injury.

Gabrielle van der Velde, D.C., B.Sc.

Dr. van der Velde is a licensed chiropractor and Ph.D. candidate in clinical epidemiology and health care research in the Department of Health Policy, Management and Evaluation, Department of Public Health Sciences, at the University of Toronto. She is a research fellow at the Institute for Work and Health in Toronto and is an associate professor in the Division of Graduate Studies and Research at the Canadian Memorial Chiropractic College in Toronto. Dr. van der Velde will have a lead role in constructing a decision analytic model of treatments for neck pain and examining neck pain patients’ preferences for various treatments, in addition to the duties shared by all members of the Scientific Secretariat
